Loading Value in Card

Load a monetary value onto a Shinagawa Premium Card by visiting Shinagawa Clinic Branches.
The value of your Shinagawa Premium Card is subject to the following load limits:

Shinagawa Premium Card Point System

VALUE	POINTS EARNED
PHP 1,000.00 – PHP 4,999.00	1 point earned every 100 spent
PHP 5,000.00 – PHP 9,999.00	2 points earned every 100 spent
PHP 10,000.00 and above	3 points earned every 100 spent

• Points can only be credited to the members who paid for the procedure.
• The points on the card can only be used to redeem non-surgical aesthetic procedures.
• Points can be earned on top of the promos or discounted prices.
• Peso value has no expiration
• Points will expire after one year from the date the last point was earned.

Terms & Conditions

• The Shinagawa Premium Card is issued to you by Shinagawa Lasik & Aesthetics. Only one Shinagawa Premium Card is allowed per client per transaction.
• Shinagawa Premium Card is considered as equivalent to cash. To earn points, the member should present his/her card prior to settlement of his/her bill.
• The value loaded onto this card is non-refundable and may not be exchanged for cash and credit balance unless required by law.
• No valid Premium Card presented, no points granted.
• Use of this card constitutes acceptance of our terms and conditions.
• Lost, stolen or damaged card must be reported to us immediately at (+632) 7-368-5238. In such case, the card will be replaced with a new one with a replacement fee of Php 200.00.
However, the earned points and remaining balance on the lost, stolen or damaged card will no longer be restored.
• Shinagawa Lasik & Aesthetics reserves the right to modify or discontinue the program at any time without prior notice.
