Promo Mechanics:
1. LASIK screening and procedure period shall be from July 1-20, 2025 ONLY. Slots are limited per day.
First book, first serve policy applies.
2.Both patients must schedule their screenings and surgeries on the same day, and must both be qualified
for LASIK to avail.
3. Promo is applicable in both Shinagawa branches (BGC and Ortigas).
4. Applicable for cash/straight payments and credit card installment payments with 0% interest for up to
24 months. Available tenors will depend on the bank, and a minimum transaction amount is required
per tenor.
5. If patients are recommended/required different LASIK procedures after the screening, they will pay the
procedure with the higher amount. The procedure with the lower price will be the one to be considered
free.
6. The ₱15,000 discount will only be applicable if both referrals undergo the SAME LASIK procedure, and it
will be deducted from the total price.
7. If only one (1) patient will push through with the procedure, the 50% off + P10k off discount will be applied instead.
8. Appointments must be booked in advance via the Shinagawa Rewards Circle website ONLY. No walk-ins
allowed. A total of (2) referral forms will be submitted on the referee’s account.
9. To avail the promotional rate, use SRCDUO2025 in the promo code section upon submitting the referral forms.
10. The promotional discount or free item(s)/services(s) is not convertible to cash, discount or other
item(s).
11. The offer cannot be exchanged for other items or discounts and cannot be used in conjunction with
other promotional offers.
12. The referee may earn an additional amount, subject to the applicable reward tier, on top of the current reward upon the successful surgery of two (2)
referrals. If only one (1) referral proceeds, no additional amount will be granted, only the regular reward will apply.
13. In the purchase of goods and services which are on a promotional discount, the senior citizen or
person with disability can avail of the promotional discount under this Promo or the discount provided
under the Expanded Senior Citizens Act for 2010 or Magna Carta for Disabled Person, whichever is
higher.
