2. Patients must undergo comprehensive eye screening at Shinagawa and final evaluation of an ophthalmologist to determine eligibility for LASIK.
3. Prior appointment is required. No walk-in allowed. Fill out the registration form below.
4. Company ID must be presented on the day of the appointment
5. Booking and surgery period to avail promo is from Nov. 16-30 2022 ONLY.
6. Atleast 2 patients must qualify and proceed with the LASIK procedure together to avail plus one treat for another patient.
7. Third patient who will be rendered with Free procedure may be scheduled until December 15, 2022 only and must be claimed in the same branch. Black out dates apply (Saturdays and Holidays)
8. Third patient who will claim the free procedure must present the official receipt and signed acknowledgment on the day of surgery.
9. If third patient is not qualified for LASIK procedure, full screening fee will apply and the free LASIK procedure will still be transferrable.

12.
