Guidelines:

1. Patients must be 18 years old and above.
2. Must undergo comprehensive eye screening at Shinagawa (at no additional cost) and final evaluation of an ophthalmologist to determine eligibility for LASIK or other procedures.
3. Prior appointment is required. No walk-in allowed. Shinagawa will contact the UBP employee after he or she completes and submits the registration below.
4. Company ID must be presented on the day of the appointment.
5. Can be extended to the immediate family of the employee. Employees must accompany the patient and present the company ID.
6. Discounted rate is for LASIK, ICL and Cataract surgeries only. Other treatments or procedures are not included.
7. Cannot be combined in conjunction with other promotional discounts or offers.
8. Discounted rate is applicable for cash, credit card, and other e-wallet payments (e.g. GCash and via UnionBank online).
9. LASIK surgery fees are inclusive of a 5-year warranty, post-op kit and medicines, follow-up check-ups, and consultations.
10. ICL and Cataract surgery fees are inclusive of RT PCR test, post-op kit and medicines, follow-up check-ups, and consultations.
